Confidentiality Policy

University Employee Health Clinic (UEHC) values your privacy, and we are committed to protecting your personal information.  UEHC will not disclose information except as required by law or when there is a threat to the health and safety of employees or volunteers.

Release of Information

You may request specific information from UEHC. You can also view and print your own immunizations and TB screening records by signing in to the UI Human Resources website: https://erma.healthcare.uiowa.edu/uihchr/index.page

UEHC does not fax information to external fax machines because of data security concerns. UEHC will, however, accept medical information via fax: 319-384-9697.

To request a copy of your records from UEHC, please include your name, birth date, information you are requesting (such as immunization or TB information), and an address for where it is to be sent.  UEHC will mail the requested information if you are no longer employed here, or you can pick up the requested information at the UEHC reception desk during work hours.  If there are multiple requests within a year, there is a minimal fee payable by check only to cover copying costs.

Request for copies of medical records from University of Iowa Hospitals and Clinics should be directed to the Release of Information Office at 319-356-1719, Monday through Friday from 8 a.m. to 5 p.m.

Compliance Status

UEHC may share your compliance status with your supervisor to let them know if policy requirements have been met. We will not disclose specific diagnoses or other medical information such as lab test results. This is in accordance with the Health Information Portability and Accountability Act (HIPAA).

Departmental Human Resource Personnel will monitor new employee compliance. This information is displayed on the HR database, ER3MA. ER3MA displays individual employees’ compliance with a “Health Screening- Initial Physical” field when a person has started their health screening process. When everything is completed, a completion date is entered in “Health Screening-Initial Physical” field. If incomplete, the individual’s department will follow up with the employee to ensure appropriate compliance requirements are met.

ER3MA database records the date when an employee has completed the required screening/surveillance evaluation. The specific results are not posted on this database, but are individually discussed with the person at the time of their evaluation.
