First Year

The goal of our residency is to train competent physicians skilled in providing continuing and comprehensive personal care to patients and their families. During their first year, residents will become competent to provide care to the hospitalized patient through rotations on our Family Medicine Inpatient Service, SNICU and the ETC. Residents will begin to provide care to patients in their continuity clinic at least weekly. At the end of the year, residents will be competent to supervise others and to act with limited independence.

Resident at Work

Introduction to Family Medicine
All incoming residents participate in the "Introduction to Family Medicine" July rotation. In addition to providing the residents orientation information to the program and the institution, the residents participate together in ACLS, PALS, NRP courses, procedure labs and community activities. A High Challenge Ropes Course is held at the end of the month with the goal of developing team-building skills, which are utilized throughout the training program.
Family Medicine Inpatient Service
The Family Medicine Inpatient Service is located at University Hospitals and is comprised of two junior residents, a senior resident (PGY2 or PGY3), often a Sub-I (senior medical student), and a faculty-attending physician. There are also two "Night Float" residents (senior and junior) who provide overnight coverage during the week along with an "On-Call" resident who covers weekends. The service admits patients of all ages with a wide variety of medical and surgical problems from the Family Medicine Clinic and referring physicians from the University of Iowa Community Medical Services Family Care Clinics. Patients are cared for within the general medicine, surgical or pediatric wards, and the telemetry unit. The inpatient team also provides medical consultation to surgical, medical and psychiatric services at UI Hospitals and Clinics.
Family Medicine Inpatient Service - Night Float
Residents in this position will assume care of all Family Medicine inpatients, including newborns, post-partum women, and any children, for the evening and overnight hours during the week, and after the inpatient team has rounded on the weekends. Residents work in pairs with a junior and senior resident for this 2-week rotation.
Family Medicine COC Clinic
During the first year, residents will spend 1-2 half days each week in their Continuity of Care Clinic. It is in this setting that residents develop their office medicine skills and provide care to patients who selected them as their family physician. Residents maintain responsibility for their patients' care throughout the three years of training, thereby acquiring the skills needed to deliver continuing and comprehensive care. Emphasis is placed on patient education and maintaining health as well as treating disease.
Neonatal Intensive Care Unit
Residents work in our Level IIIC NICU with a team of neonatal nurse practitioners, PGY-2 pediatric resident and a staff neonatologist in Bay 2-3 providing care to critically ill infants. Residents also will be called to the delivery room and operative room for all newborn resuscitations.
Pediatrics Clinic
Residents participate in the General Pediatric diagnostic clinic, growth clinic, and encopresis clinic, as well as manage acutely ill pediatric walk-in patients of all ages. During this rotation, residents have the opportunity to work with Pediatric faculty and residents.
Surgical and Neuroscience ICU
The Surgical and Neuroscience Intensive Care Unit is the major academic referral center for support of critically ill patients in Iowa. The Surgical and Neuroscience Intensive Care Unit is a closed unit and is medically directed and staffed by faculty members of the Department of Anesthesia and Surgery. Residents provide care to patients with respiratory failure, sepsis, multi-system trauma, peri-operative complications, acute neurological injuries, and post-organ transplantation care.
Psych Consult
Residents work with Psychiatry residents and staff as an inpatient hospital consult service evaluating psychiatric disturbances that occur among the medically ill. The Consultation team often has to deal with legal problems as well, including capacity evaluations and legal 'hold' issues (if a patient needs to be psychiatrically hospitalized against their will). You will also evaluate outpatient consultations, which include everything from depression to anxiety to dementia.
Emergency Medicine
Residents rotate through the Emergency Treatment Center, a Level I trauma center, under the supervision of Emergency Medicine faculty. This training includes formal didactic presentations provided by the Emergency Medicine faculty during the rotation as well as critical care lecture series given by faculty with joint appointments in Family Medicine and Emergency Medicine. Residents participate in focus learning courses in Advanced Cardiac Life Support, Pediatric Advanced Life Support, Neonatal Resuscitation, and Advanced Trauma Life Support.
Chronic Disease
Chronic disease has become the dominant health problem in the US, accounting for over 75% of healthcare spending. This rotation helps to prepare our residents to manage patients with chronic diseases. You will spend time in specialty clinics and learn different approaches to systematic care for chronic illnesses. For example, some clinics utilize a case manager, while others provide a multi-disciplinary team approach to providing care. Residents become familiar with these clinical tools in order to incorporate them into their practice. Residents will also have the opportunity to develop skills in providing care to patients with severe and persistent mental illness (SPMI) through a million dollar HRSA grant our residency program has received. This rotation occurs in January with all PGY-1 residents and includes group didactics on chronic disease, wellness and motivational interviewing.
Trinity Obstetrics
Residents participate in an Obstetrics rotation at Trinity Medical Center, Moline, IL - 50 miles east of Iowa City. Supervised by board-certified obstetricians and midwives, residents learn the principles and techniques of prenatal care, management of labor and delivery and postpartum care. The residents are provided an apartment and a car for transportation while on the Trinity rotation.
Longitudinal Behavioral Medicine
Knowledge and skills in behavioral medicine and psychiatry are integrated throughout the residents' educational experience. Training is primarily in the outpatient setting through a combination of longitudinal experiences and didactic presentations. The behavioral medicine and family physician faculty members work closely with the residents to develop and refine physician/patient relationships, patient interviewing skills and counseling skills in the Family Medicine Clinic.
Longitudinal Geriatrics
Care of the older patient is provided throughout the residency in the care of assigned patients in the Family Medicine Clinic and Family Medicine Inpatient Service settings. Residents also participate in Geriatric Clinics and Nursing Home Rounds with family medicine and internal medicine faculty, and meet with the Geriatrics Multidisciplinary Team to discuss unique problems of their geriatric patients. These longitudinal and focused learning experiences provide residents educational experiences in common and complex clinical problems of the older patient, including preventive aspects of health care, the acute and chronic entities of aging, and the effective use of all members of the health care team. Seven members of the Family Medicine faculty have Certificate of Added Qualification (CAQ) in Geriatrics and participate in the Geriatrics Fellowship.
Longitudinal Management of Health Symptoms
Practice management has become increasingly important in the training of family physicians, regardless of the mode of practice within the specialty. The curriculum provides a broad economic, legal, human and conceptual foundation so as to prepare residency graduates to ask sound business questions, interpret financial data, and promote and develop a productive work environment in the practice setting. Curriculum is presented throughout the three years in the form of seminars, workshops and practical experience in the Family Medicine Clinic and independent study. Members of the University of Iowa business and law faculty and experts from statewide and community health care settings give presentations on finance and accounting, legal issues, human resources and personnel issues, effects of legal and economic environmental trends, leadership perspectives, marketing and technology in the office.
