Short Title
UI Asthma/COPD Registry
Official Title

UI Asthma/COPD Registry

This will establish the UI Asthma and COPD Registry of future research participants. The purpose of this registry is to identify individuals who have an interest in participating in research studies conducted by researchers at the University of Iowa.

If or when there is an appropriate study for your indicated condition, you will be contacted by phone by a member of the research team with information about the study. The purpose of the contact will be to let you know about a study and see if you are interested and eligible. The number and type of study volunteers needed each year varies, so we cannot guarantee that you will be contacted or enrolled in a study.

Participation in any study is voluntary, and submitting your name to the registry in no way obligates you to respond to any requests for study volunteers.

If you have asthma, COPD, emphysema, or you are interested in being a healthy control subject for pulmonary (lung) research studies, we would like you to provide information about yourself including your birth date, gender, diagnosis, and contact information.

We will keep information you provide confidential. However, federal government regulatory agencies, auditing departments of the University of Iowa, and the University of Iowa Institutional Review Board (a committee that reviews and approves research studies) may inspect and copy records pertaining to this research.

Only members of our research team who work with Dr. Kline will have access to this registry. Registry information is kept in a password protected database. The information you provide for us will only be used for the purpose of contacting you for possible future research. Your contact information will not be sold or distributed to any outside party or institution.

There are no risks from being in this registry and you will not benefit personally. you will not have any costs for being in this registry. You will not be paid for being in this reg
