To receive W2 tax forms, update your home address prior to leaving the Iowa City area at Employee Self-Service. Access to Employee Self-Service will continue for generally 18 months following the completion of your training program.

UI Alumni Association
Upon completion of your training program at UI Hospitals and Clinics, residents and fellows become alumni of the University of Iowa and are eligible for benefits provided by the UI Alumni Association. One benefit is access to an online directory for keeping your location and accomplishments up-to-date. Sign up for this service on the UI Alumni Association website.
National Provider Identifier (NPI)
The Center for Medicare and Medicaid Services (CMS) requires a unique identification number for each healthcare provider. This identification number is called a National Provider Identifier (NPI). Incorrect provider demographic information found in the NPI Registry can delay contact/clarification from other healthcare providers, home going agencies, and insurance companies with the prescribing provider that can adversely affect patient care. Therefore, keeping your NPI demographic information current is a requirement of CMS as outdated information can permanently adversely affect your eligibility as a Medicare participant. Update your demographic information within 30 days of a professional move.

Medicare requires all physicians to maintain current demographic information. When a physician moves, demographic information must be updated within 30 days. Update your demographic information on the NPPES website.
