How to Refer a Pediatric Patient

As a health care professional, you may refer a pediatric patient to UI Children’s Hospital using one of the following methods:

• Refer online

  Our online referral form uses basic encryption that lets you initiate a referral. To protect confidential records, a fax or phone follow-up will be required.

• Refer via fax

  Faxed referrals forms are secure and may contain confidential information. We will follow up with you by telephone to obtain any information needed to complete the referral.

• Refer via telephone

  Call our 24-hour UI Consult line at 800-322-8442 to begin a referral.
